Which Area Agency on Aging serves each county?

INCOG serves Tulsa, Creek and Osage counties at (918) 584-7526; Grand Gateway serves Rogers County at (918) 783-5793; and EODD serves Wagoner and Okmulgee counties at (918) 682-7891.

All six counties follow the same Oklahoma licensing rules, SoonerCare and ADvantage policies and statewide hotlines; what changes at the county line is the local agency, the ombudsman office and, for Wagoner County, PACE options.

Which county has the oldest population?

Osage County: 23.4% of its residents were 65 or older in 2025 Census estimates, with a median age of 43.9, compared with 16.6% in Tulsa County and 18.0% statewide.

Yet Osage County has few licensed beds and only small critical access hospitals, so many Osage families weigh care in Tulsa, Skiatook or Sand Springs against staying home with help.

Where is most of the region's licensed senior care?

Overwhelmingly in Tulsa County, which holds 39 of the 45 assisted living centers and 33 of the 55 nursing homes on OSDH's roster for the six counties.

Each county page lists every licensed facility and the straight-line distance from the county seat to the nearest hospitals and to downtown Tulsa.

How many older residents live in the six counties?

Census Bureau estimates put 187,288 residents aged 65 and older in the six counties on July 1, 2025, including 19,518 aged 85 and older, the group most likely to need daily help.

Tulsa County holds most of those older residents in absolute numbers, but the outer counties have higher shares of residents 65 and older and far fewer licensed beds, which is why families there so often compare local options with facilities in Tulsa, Broken Arrow, Owasso or Claremore.
