Where does the directory's data come from?

Only from official bulk data: OSDH's Long Term Care public survey system (facility roster and each facility's public-file index, retrieved October 31, 2026), CMS's Provider Information dataset for nursing home ratings, inspections, staffing and fines, and CMS's hospital list for distances; we do not use aggregator listings, operator marketing or reviews.

Every listing page carries a “Source” line with the data date. 106 closed facilities in the six counties are excluded. Facility pages exist for every licensed assisted living center and every CMS-matched nursing home; residential care homes and adult day centers appear as roster rows.

How do I use the listing pages?

Each listing page has a search box and filters for county, city and license type (and CMS rating for nursing homes), and every column can be sorted by clicking its heading, so you can, for example, list every assisted living center in Broken Arrow by licensed beds.

What do the survey dates on the directory mean?

The survey date is the exit date of the newest survey document in the facility's OSDH public file; Oklahoma's rule calls for an unannounced assisted living inspection at least every 15 months, so an older date is a reason to ask the facility for its latest report.

Across the 45 licensed assisted living centers, the median gap between the newest survey document and this build is 12 months. Public files can lag behind actual visits, so treat the date as a prompt for a question, not as proof that an inspection was missed.
