TL;DR

Wagoner and Okmulgee counties are served by the EODD Area Agency on Aging in Muskogee, (918) 682-7891. Licensed care is mostly nursing homes; Okmulgee has one small assisted living center and Wagoner County none, so families often look to Broken Arrow or Tulsa for assisted living.

Who serves these counties

Wagoner County, east and southeast of Tulsa, had about 93,263 residents in 2025 Census estimates, including Coweta, the town of Wagoner and the Wagoner County side of Broken Arrow. Okmulgee County, south of Tulsa, had about 37,241, with Okmulgee, the capital of the Muscogee (Creek) Nation, as its seat and Henryetta, Morris and Beggs among its towns. Both belong to the Eastern Oklahoma Development District's Area Agency on Aging in Muskogee, (918) 682-7891, not INCOG.

That matters for in-home services, caregiver programs and help with ADvantage applications, which run through the Area Agency on Aging. Wagoner County residents also have a PACE option to check: Cherokee Elder Care PACE in Tahlequah lists Wagoner County ZIP codes in its service area.

Coweta Care & Rehab Center holds a five-star CMS overall rating in the data we use. The Wagoner residential care license is attached to the Wagoner nursing home. In Okmulgee County, the Neighborhoods at Baptist Village of Okmulgee is the only assisted living center on the roster, a small one.

Assisted living means a drive

With no assisted living center in Wagoner County on OSDH's roster and one small center in Okmulgee County, most families in these counties who want assisted living look at Broken Arrow, which has the largest group of centers outside Tulsa, or at south Tulsa. Coweta families are typically a short drive from Broken Arrow's centers. Henryetta and Okmulgee families face a longer trip and sometimes choose a local nursing home instead, especially when nursing needs are significant.

Hospitals and health care

CMS's hospital list includes Wagoner Community Hospital, Muscogee (Creek) Nation Medical Center in Okmulgee and Hillcrest Hospital Henryetta. For Muscogee (Creek) Nation citizens, tribal health services may be part of the care plan, and it is worth asking the tribe's health system what elder services it offers alongside state programs.

Checking a facility from the state file

Every licensed facility in both counties has a public file on the OSDH survey site, surveys.health.ok.gov. Read the newest survey document, then the one before it, and look for problems that repeat. For nursing homes, add CMS's Care Compare data, which our facility pages reproduce with the Tulsa-area medians beside each figure. In a small county where there are only one or two realistic options, that homework is the best tool a family has, because the usual advice to compare five buildings does not apply.

Visit at mealtime and in the evening, ask how many staff are on each shift, and ask who you call at night if something goes wrong. A good administrator in a small home will usually give you a direct phone number.

Getting help with the paperwork

EODD's Area Agency on Aging can help with ADvantage applications and connect families to local in-home services. The Medicare Assistance Program at 1-800-763-2828 counsels on Medicare plans, and the long-term care ombudsman at 1-800-211-2116 helps residents and families resolve problems once someone lives in a facility.

Planning for a later move

Because the nearest assisted living for many families here is in Broken Arrow or Tulsa, a parent who starts with help at home in Coweta, Wagoner, Okmulgee or Henryetta should know where the next step would be before it is needed. Tour one or two centers early, ask whether they accept ADvantage, and keep the list current. If nursing care becomes necessary first, the local nursing homes in Wagoner, Coweta, Okmulgee and Henryetta keep a resident close to family and church, which counts for a great deal. Read each home's CMS record before a crisis forces a fast choice.

Making home care work

Many older adults in these counties stay home with help. ADvantage pays for personal care and other services at home for those who meet nursing facility level of care and the financial limits, and EODD administers Older Americans Act in-home services. Apply early for ADvantage because of the waiting list, and ask EODD what in-home help is available while the application is pending.
