Creek and Osage counties have older populations than Tulsa County (20.6% and 23.4% aged 65 and older) but far fewer licensed beds. Most licensed care there is nursing homes; assisted living is limited, so families often combine local nursing homes with assisted living in Sand Springs, Glenpool or Tulsa.
Two counties, one Area Agency on Aging
Creek County, along old Route 66 southwest of Tulsa, had about 74,967 residents in 2025 Census estimates, with Sapulpa as its county seat and Bristow, Drumright, Mannford and Kiefer among its towns. Osage County, the reservation of the Osage Nation and Oklahoma's largest county by area, had about 46,794, with Pawhuska as its seat and Skiatook's western side, Hominy, Barnsdall and Fairfax among its communities. Both are served by INCOG's Area Agency on Aging at (918) 584-7526, the same agency as Tulsa County.
They are older than the urban core. Osage County's median age is 43.9 and Creek County's 40.9, against 36.8 in Tulsa County.

Nursing homes do most of the work
Outside the edge of Tulsa, licensed care in these counties is mostly nursing homes in small towns: Drumright, Mannford, Barnsdall, Skiatook, Fairfax and Sapulpa. CMS ratings vary widely among them, from one star to five, so read the inspection and staffing details before choosing. Small-town homes often have long-tenured staff and a strong community feel, and they also tend to be the only option within a reasonable drive, which makes the CMS data more important, not less.
Finding assisted living nearby
Sapulpa has no assisted living center on the state roster. Families in Sapulpa and Kiefer usually look at Glenpool's small residential-style homes and the large centers in south Tulsa; families in Mannford and Sand Springs look at Green Tree at Sand Springs, which has a dementia disclosure on file. Osage County families in Skiatook and Sperry often look north and east to Owasso and Collinsville. Distance matters: a parent who gets frequent visits is better protected than one who does not.
Questions for a small-town nursing home
Small homes deserve the same scrutiny as large ones. Read the latest CMS inspection, check registered nurse hours and turnover against the Tulsa-area median, and ask how the home handles after-hours medical problems when the nearest hospital is a critical access hospital or a drive to Tulsa. Ask whether the medical director visits in person, how often a nurse practitioner sees residents, and how residents get to specialist appointments. Ask about staffing on weekends and holidays, when rural homes are most stretched.
Tribal health services
Osage Nation and Muscogee (Creek) Nation citizens, and citizens of other tribes living in the area, may have access to tribal health and elder services in addition to state programs. Ask the tribe's health system what elder services, home care or transportation it offers, and how they coordinate with SoonerCare and Medicare.
Respite and adult day options
Caregivers in small towns often have fewer breaks. The adult day center in Sapulpa offers daytime care, some assisted living centers in Sand Springs, Glenpool and Tulsa accept short respite stays, and INCOG can tell families about caregiver support and in-home respite funded under the Older Americans Act. Plan respite before the caregiver is exhausted; it is easier to arrange when it is not an emergency, and it gives the family a trial run of a setting it may need later.
Help at home
For older adults who want to stay in a small town, help at home is often the realistic plan. ADvantage can pay for personal care at home for those who qualify, and INCOG administers Older Americans Act in-home services in both counties. The adult day center in Sapulpa offers daytime care. Hospitals are smaller here too: Ascension St. John Sapulpa, Drumright Regional, Carrus Lakeside in Bristow, Pawhuska Hospital and Rural Wellness Fairfax are on CMS's list, so specialty care usually means a trip into Tulsa.
