Who inspects assisted living in Tulsa?

OSDH's Long Term Care Service inspects every assisted living center, unannounced, at least every 15 months, with a statewide average of 12 months.

Results are filed as survey documents in each center's public file at surveys.health.ok.gov.

Is assisted living the same as a residential care home in Oklahoma?

No: assisted living centers may provide intermittent nursing and medication administration, while residential care homes provide room, meals and supportive assistance without medical care.

They are licensed under different rules, OAC 310:663 and OAC 310:680.

What is a continuum of care facility in Oklahoma?

It is a campus licensed to provide both assisted living and nursing home care, so residents can move between levels without leaving the campus.

OSDH gives the assisted living part its own license ID, often the nursing home ID with AL added.

Does Oklahoma rate assisted living centers?

No; neither OSDH nor CMS publishes star ratings for assisted living, so the survey documents in the public file are the main official record.

Only certified nursing homes have CMS star ratings.

How do I know an Oklahoma facility's license is current?

Look the facility up on the OSDH survey site or in our directory; every listing in our directory comes from OSDH's roster of open facilities.

Closed facilities are excluded from our roster.

What happens to a new assisted living center before its first inspection?

It operates on an initial license until OSDH completes a full inspection; if the center is not in compliance then, the initial license can be extended once for up to 120 days.

Ask a new center when its full licensing inspection took place.

Are assisted living administrators licensed in Oklahoma?

They must hold a nursing home administrator license, an assisted living and residential care administrator certificate of training, or an approved national certificate, and complete 16 hours of continuing education a year.

The OSDH roster lists each facility's administrator of record.

Who licenses home care agencies in Oklahoma?

OSDH licenses home care agencies and hospices under separate programs from the long-term care facilities in our directory.

Check any agency's license with OSDH before hiring.

Does Oklahoma license independent living communities?

Not as care facilities: independent living apartments that provide no personal care need no assisted living license, although the same campus may hold an assisted living or nursing home license for its care units.

Ask which parts of a campus are licensed and under which license ID.

What does the AL or CC in an Oklahoma license ID mean?

OSDH license IDs carry a prefix for the facility type, such as AL for assisted living, RC for residential care, DC for adult day care, NH for nursing homes and CC for continuum of care campuses.

Use the ID to search the OSDH survey site.
