How do I file a complaint about a Tulsa assisted living center?

Call OSDH's long-term care complaint hotline at 1-800-747-8419.

Complaint investigations appear later as survey documents in the facility's public file.

Who do I call about elder abuse in Oklahoma?

Oklahoma Human Services' abuse and neglect hotline at 1-800-522-3511, or OKHotline.org; call 911 if someone is in danger.

You only need a reasonable concern to report.

What is the Oklahoma long-term care ombudsman number?

1-800-211-2116, the statewide line for the program now run by the Oklahoma Attorney General's Office.

In Tulsa, Creek and Osage counties, INCOG supervises local ombudsmen (918-579-9423).

Can an assisted living center make my parent leave?

A center may not keep a resident whose needs exceed its licensed level or capabilities, and its discharge criteria must be in the service contract.

Ask the ombudsman for help if you disagree.

What notice must a nursing home give before discharge?

Federal rules for certified nursing homes generally require 30 days' written notice with appeal rights, except in specific urgent situations.

Contact the ombudsman as soon as a notice arrives.

Are nursing home inspection results public in Oklahoma?

Yes, through the OSDH public file and, for certified homes, CMS Care Compare.

Our nursing home pages reproduce CMS's figures.

Where do I complain about a home health agency?

OSDH's home health (Medicare) hotline is (800) 234-7258.

Hospice and home care agency complaints also go to OSDH.

Can families see a facility's inspection results?

Yes; OSDH posts survey documents in each facility's public file online, and certified nursing homes must also make their most recent survey available on site.

Our facility pages link the latest dates to the OSDH file.

Who protects a resident's money in a facility?

Residents can manage their own money or designate someone; if a facility holds funds, ask how they are accounted for, and report suspected financial exploitation to 1-800-522-3511.

The ombudsman can help with disputes over charges.

Can I complain anonymously in Oklahoma?

Yes; OSDH's hotline and the ombudsman accept concerns without requiring you to give your name, though a contact helps investigators follow up.

Ombudsman assistance is confidential.

What if a facility does not respond to a complaint?

Escalate: the ombudsman can advocate directly, OSDH can investigate rule violations, and Adult Protective Services handles abuse, neglect or exploitation.

Keep dated notes of every contact.

Can a resident refuse care or medications?

Generally yes; residents keep the right to make their own decisions, and the facility should document the refusal, explain the risks and involve the physician and family as appropriate.

Ask the facility how it handles refusals for residents with dementia.
